Group Application
Please complete a Group Application if your practice has 2 or more providers filing under the same Tax ID Number. Each provider within the group will also need to complete the Provider Application.

Corphealth participates in CAQH as a subsidiary of Humana. In order for Corphealth to access your CAQH application, please give Humana or "all health plans" authorization on the CAQH website.

The following states require state specific provider applications. Except for Arkansas physicians, include copies of documents listed on the Checklist:

Once Corphealth has received a completed application and signed contract the credentialing process begins. The following information is verified:

License(s)– must be current and unencumbered, require at least a Master’s degree, and be able to practice independently

Malpractice Insurance– minimum limits of 1M/3M for physicians; 1M/1M for non-physicians

Education

Hospital Privileges (if applicable)

Board Certification

Physicians:
Unless date of birth is prior to 1950, Physicians must be Board Certified by ASAM in Addiction Medicine or Certified by the ABMS or AOA in Psychiatry OR completed a residency in Psychiatry and be progressing toward certification within 5 years of initial credentialing.

APN/CNS:
Advanced Practice Nurses/Clinical Nurse Specialists must have state or national mental health certification.

 

The National Practitioner Data Bank is queried for information regarding malpractice settlements, license suspension, and Medicare sanctions. A written response is requested for any affirmative answers on the provider application or from the National Practitioner Data Bank. The final step is a review of the provider application by the credentialing committee. The entire credentialing process can take up to 120 days to complete. Providers will be notified by mail once the process is complete.
