How do I go about joining Corphealth's network?

You may download a Provider Application by clicking on the link at the left. Complete the application, attach the required documents, and mail to the address indicated on the application. Your application will be forwarded to Corphealth’s Credentialing Dept. The process takes approximately 60 days. When all of your information is complete, your application will be presented to the Credentialing Committee for approval. You will be advised of Credentialing Committee action.

How do I check the status of my application?

Call Corphealth’s Provider Relations Department at 1-800-890-8288.

What are your fees and how do you arrive at them?

Corphealth fees are based on market rate conditions as determined by our surveys, and comments from providers, purchasers, and members. A fee schedule accompanies the provider application.

How much paper work is there?

Corphealth's Provider Initiative is designed to simplify the certification process. For outpatient behavioral health, use the Concurrent Outpatient Clinical Review Form, which is one page in length. You may download this form from this web site and fax it to us at (800) 807-1898.

How many outpatient visits will you authorize at a time?

Initially, we will authorize 8-10 visits for outpatient mental health treatment. EAP visits depend on the particular contract. Assess and refer programs range from 1-3 sessions. Assessment and treatment can range from 1-6 or 1-8 sessions.

How many lives do you have (especially in my area)?

Corphealth is constantly growing its business nationally. If you are receiving a new application, there is a good chance that Corphealth is developing the network in your area, and that we expect new business to come on line. Who are the payors for Corphealth?

Corphealth's payors include insurance companies, employers, and government agencies. We handle mental health benefits and employee assistance programs as well as a product called Balance where Corphealth manages, for the same payor, both its mental health and EAP benefit. If you are a Corphealth provider, you are eligible to serve any of these payors. Contracts vary from community to community, but our Provider Relations Department can provide you with the specifics for your area.

What are the benefits for a Corphealth member?

Benefit structures vary from contract to contract, and members are often unsure of what their benefit entails. Benefits are designed by purchasers in collaboration with health care consultants. Corphealth is hired by the purchaser to administer its behavioral health care benefits. Our certification letter provides you with some important information, and you can get additional information by calling the Corphealth Helpline at (800) 777-6330.

Why haven't I received any referrals lately?

In some situations, members are already seeing a specific provider. In this case, we do not attempt to "steer" the member to someone else. In other situations, our case managers may not know that you are available to accept referrals. Please feel free to contact our Clinical Case Management Department at (800) 777-6330 and ask to speak to a supervisor to let them know of your interest and availability. Another action to take -- check the accuracy of your listing in our Provider Look-up on this web site. If you have a correction, please e-mail your correction to providerrelations@Corphealth.com.

Where do I send claims and clinical OTR forms?

For all accounts, except Arkansas, you may fax claims to 817 334-0397. OTRs and other communication with the Clinical Department can be faxed to 800 807-1898. You may also mail claims to Corphealth, 1300 Summit Ave., Suite 800, Fort Worth, TX, 76102-4420.

Arkansas claims and OTRs may be faxed to 501 217-8075. Arkansas claims and forms may be mailed to Corphealth, STAR EAP, 1701 Centerview Dr. Suite 101, Little Rock, Arkansas, 72211.

How do I check the status of my credentialing or recredentialing application?
