Members Rights
 
 

Corphealth members have the following rights and responsibilities related to the care they receive. Treatment and the quality of care must be provided to members without regard to their race, sex, religion, marital status, sexual orientation, age, national origin, or ancestry. Clear communication of expectations regarding the member's responsibilities is very important.

Members have the right to...

Be treated with respect and recognition of their dignity and need for privacy.

Receive information concerning Corphealth services and practitioners, clinical guidelines, and client's rights and responsibilities.

Be provided with information regarding the cost of care (such as co-payments and deductibles) and the factors that may affect the cost of care.

Receive a response to questions or concerns in a prompt and courteous manner.

Participate with practitioners in decision making regarding their treatment planning.

Receive complete information and explanation of any diagnosis, proposed treatment or treatment alternative, including non-treatment, in order to give informed consent.

Be an active participant in the decisions related to care.

Be informed about the complaint/grievance procedures available.

Voice complaints or appeals about Corphealth, their provider, or the care provided.

Be informed of the names, specialties, and qualifications of Corphealth providers.

Be able to change provider by discussing this with their therapist and by contacting Corphealth's toll-free phone number.

Make recommendations regarding Corphealth's rights and responsibilities policy.
A candid discussion of appropriate or medically necessary treatment options for their conditions, regardless of cost or benefit coverage.

Members have the responsibility to...

Know how to access services on either a routine or emergency basis.
Contact Corphealth's toll-free phone number with questions, concerns, or for an authorization number if needed for treatment.
Supply information (to the extent possible) that practitioners and providers need in order to provide care.
Provide necessary, accurate information to Corphealth so appropriate services may be provided.
Follow the plan and instructions for care that they have agreed upon with their providers.
Participate, to the degree possible, in understanding their behavioral health problems and developing mutually agreed upon treatment goals.
Inform the provider about changes in medications, lifestyle, or functional status which could impact treatment.
Inform provider or staff of changes in insurance coverage, employment status, address, phone numbers, or emergency contact.
Treat office staff courteously in person and on the phone.
Inform the office staff if they plan to use an EAP benefit before accessing their mental health benefit.
Provide accurate and truthful information to the therapist.
