receive. Treatment and the quality of care must be provided
to members without regard to their race, sex, religion,
marital status, sexual orientation, age, national origin,

clinical guidelines, and client's rights and responsibilities.

regarding the cost of care (such as co-payments
and deductibles) and the factors that may affect
the cost of care. |

Receive a response to
questions or concerns in a prompt and courteous
manner. |

Participate with practitioners
in decision making regarding their treatment planning.

Receive complete information
and explanation of any diagnosis, proposed treatment
or treatment alternative, including non-treatment,
in order to give informed consent. |

Be an active participant
in the decisions related to care. |

complaint/grievance procedures available. |

care provided. |

Provide necessary, accurate
information to Corphealth so appropriate services

Follow the plan and instructions
for care that they have agreed upon with their providers. |

Participate, to the degree
possible, in understanding their behavioral health
problems and developing mutually agreed upon treatment

Inform the provider about
changes in medications, lifestyle, or functional

Inform provider or staff
of changes in insurance coverage, employment status,
address, phone numbers, or emergency contact. |

Treat office staff courteously
